Are you an experienced advanced nurse/clinical practitioner (ACP/ANP)with a passion for improving patient outcomes?

New permanent role available - Come and join us one day a week. Ideally we are looking for the successful applicant to work on Wednesdays however if you are interested in the opportunity and available on a different day(s), please do get in touch as we can be flexible and there may be opportunities for additional hours/days going forward.

This is a rewarding opportunity for an experienced ACP/ANP who is passionate about improving outcomes for older adults with complex needs. You willwork as part of the Primary Care Network (PCN) multidisciplinary team, supporting the delivery of proactive, responsive, and person-centred care within community and home-based settings.supporting frailty and care home residents across the network.

In this role, you will use your advanced clinical skills to deliver proactive, person-centred care for care home residents, providing expert assessment, diagnosis, prescribing and management planning. You will work closely with GPs, pharmacists, district nurses, palliative care teams and care home staff to ensure residents receive timely, coordinated and compassionate care.

This position offers the chance to make a meaningful impact on a highly vulnerable population, helping to prevent avoidable hospital admissions, support personalised care planning and enhance quality of life for residents living with frailty and long term conditions.
