Advanced Nurse Practitioner

Purpose of the Role:

• To provide high-quality, patient-centred care as part of a multidisciplinary team, contributing to the delivery of safe, efficient, and proactive healthcare services. This includes diagnosing and managing acute and chronic conditions, and supporting health promotion and prevention initiatives.
• Working as part of the On the day acute care team managing presentations of acute illness with support of the duty GP.

Role Purpose

• The post holder contributes to the delivery of safe, effective urgent care by providing informed clinical assessment, supporting clinical decision making, and managing a defined range of presentations in general practice and community settings.
• Working with increasing autonomy but within agreed clinical boundaries, the role applies developed knowledge of minor illness, minor injury, and urgent presentations to support timely patient management.
• The practitioner enhances service capacity by undertaking structured assessment, escalating appropriately, and contributing to patient flow, continuity, and a positive care experience.
