Band 5 CAMHS Practitioner - Think Family Outreach Team The Think Family Outreach Team (TFOT) is part of an exciting pilot project designed to deliver an enhanced day provision for children, and community support for young people, and their families within the urgent care pathway. The post holder will work as part of the TFOT to support children, young people, and their families experiencing emotional distress. The aim is to provide early intervention to reduce the risk of frther escalation to Crisis and Home Treatment Teams (CRHTT) or acute hospital admission. The clinical offer is tailored to meet the individual needs of each young person and their family, with a strong focus on empowerment, resilience, and skill development. Key Responsibilities

  • Deliver clinical support as part of a multidisciplinary team.
  • Provide a range of psychoeducational interventions to support crisis management and recovery.
  • Work collaboratively with families to promote understanding, coping strategies, and emotional wellbeing.
  • Contribute to care planning and ongoing assessment of needs within the crisis pathway.
  • Support a strengths-based, family-focused approach to care.
