Community Advanced Health Care Assistant - Short Term Care Team

Come and join an exciting service within North Lincolnshire who support patients when leaving hospital that require short term care and rehabilitation. The service will provide care support and re-enablement for patients and their families across seven days a week.
The team will work in collaboration with the District Nurses, locality coordinators, discharge teams and will closely link with community therapy services to promote functional independence and rehabilitation for short term care and support needs.
The advanced community health care will undertake administration of medication and must have appropriate training to be able to do so. Training will be provided in line with local policy where this is not achieved at application stage.
Please note that evidence of the NVQ qualification will be required on the day of the interview.
