Community Practitioner - Discharge to Assess/Intermediate Care at Home - Band 6 - 37.50 hours
The Discharge to Assess team are looking for a Registered Nurse, to fulfil an exciting opportunity within the community in Stockport NHS Foundation Trust
We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries. An opportunity has arisen for a Registered Nurse - Band 6, to work flexibly within the Discharge To Assess Pathway 1 and Pathway 2/Intermediate Care at Home Team.
The Discharge To Assess element is a multi-professional service who assess patients upon discharge from hospital in the community or to Pathway 2 bedded units. We provide individualised care to meet the person's needs and facilitate transfer for onward care into the appropriate service: Health, Adult Social care or partner agencies. The Intermediate Care at Home Team provide nursing and therapy rehabilitation to patients within the Stockport community.
