GP Practice Pharmacist

We are seeking a motivated and patient focused Prescribing Pharmacist to join our clinical team. The successful candidate will play a key role in delivering high quality pharmaceutical care, supporting safe and effective prescribing, and improving patient outcomes across a range of clinical services.
The Prescribing Pharmacist will work collaboratively with GPs, nurses, and multidisciplinary healthcare professionals to assess patients, manage long term conditions, conduct medication reviews, and prescribe within their scope of practice. This role requires strong clinical knowledge, excellent communication skills, and a commitment to delivering patient centred care.
We are looking for 25-30 hours depending on experience.
