This exciting new fellow post has been developed to align with NHS England's Urgent and Emergency Care Plan to move care closer to home and reduce avoidable admissions.
Our Emergency Departments are facing unprecedented pressure with high demand and overcrowding. Our patients are increasingly complex, frailandmulti morbid making it a necessity that we address challenges with the interface between primary and secondary care. The fellowship is designed to improve patient experience for frail, complex and vulnerable patients and reduce avoidable admissions.The successful applicantwould be able to work with stakeholders to create sustainable, safe community pathways that offer evidence-based care forpatients,especially those more vulnerable and frail patients where a visit to the emergency department can be extremely challenging.TheEM residentrecruited to this role would be ableto gain experience in service delivery,managementand developing leadership skillsin dedicatedtime. The fellow will be able to lead on developing new service models and clinical governance for community care which will prepare them for senior leadership roles as consultant leaders of the future.
The Community Integration fellow is a split role: 40% of time spent working clinically in the Emergency Department and 60% of time (3 days a week) dedicated to fellowship activity.
