An exciting opportunity has arisen for an enthusiastic and experienced Learning Disabilities Clinical Assessor to join the Continuing Healthcare (CHC) Team within North West London.
This post is ideal for a Registered Learning Disabilities Nurse (RNLD) or suitably experienced Allied Health Professional who is passionate about improving outcomes for people with learning disabilities and complex health needs.
Working within a specialist multidisciplinary team, you will assess, monitor and review individuals in line with the National Framework for NHS Continuing Healthcare. You will work closely with service users, families, health and social care professionals, provider organisations and local authorities to ensure individuals receive person-centred, safe and effective care that meets their assessed needs.
The role offers an excellent opportunity to develop expertise in Continuing Healthcare, commissioning and complex care assessment while gaining experience across a range of health and social care services.
