Job description

An exciting opportunity has arisen for a committed, enthusiastic and versatile individual to work within the Unplanned Care, D2A team within the South West of Staffordshire. This fast-paced role will promote and support the discharge of individuals out of hospital and provide daily multi -disciplinary working, with a range of professionals in the social care, health, community and private sector. The service predominantly supports individuals over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities.

For this role, it would be expected that you have a good experience of completing strengths based and evidence-based assessments in line with The Care Act 2014, Mental Capacity Assessments and have knowledge and experience of Safeguarding Vulnerable Adults. It would also be expected for you to have knowledge of Continued Health Care Assessments and Funding
