The Community Therapy Team provides a high-quality, responsive rehabilitation service for adults, helping people recover following illness or injury, manage long-term conditions and improve their health and wellbeing.

Based at Tooting Works in the London Borough of Wandsworth, you will join a friendly and supportive multidisciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Rehabilitation SupportWorkersand Trusted Assessors, working together to deliver person-centred care.

This is an exciting opportunity to take professional responsibility for assessing,planningand delivering occupational therapy interventions for adults referred to the service. Working closely with service users,carersand partner organisations, you will support people to achieve meaningful rehabilitation goals, maximise independence and enhance their quality of life. You will also have opportunities to contribute to service development, qualityimprovementand service evaluation, helping to shape safe,effectiveand responsive community rehabilitation services.
