An exciting opportunity has arisen at Bodmin Community Hospital for a Progress Co-Ordinator to join the Clinical Assessment Treatment Unit(CATU) Team at Bodmin Community Hospital. You will play an important role in supporting the clinical staff in facilitating safe and timely discharge from hospital for patients.

You will help provide a seamless service for patients which will include cross boundary and collaborative working with the multi-disciplinary teamalong with colleagues in Adult Social Care and other outside agencies in the community setting.

This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills and possessexcellent communication and organisational skills.

We will offer a supportive working environment in which to continue to develop your skills, a structured in-service training programme andappraisal and supervision systems.

This post to include occasional weekends and bank holidays on a rota basis. Shift split over the week to be discussed at interview and agreed with successful applicant.
