The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for frailty and stroke patients and will provide and maintain a high standard of patient focussed care that ensures patient safety; dignity and respect are paramount to the care delivered.
The post holder will-
Provide, monitor and ensure that a high standard of safe and effective nursing care is maintained via professional actions, verbal and written skills.
With the support of senior staff be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs arising from the stroke and frail cohort.
Carry out a range of routine and non-routine tasks including medication rounds, clinical observations, planning, implementing, and evaluating care plans, assisting patients to meet their daily care needs, health promotion, venepuncture, planning discharges for patients.
Work Pattern
- You will be expected to work across 7 days including nights, weekends and bank holidays on a rostered basis.
- Day shift 07:00 19:30
- Night shift 19:00 07:30

Essential: - BSc or equivalent experience in adult nursing role; - Current NMC registration
Essential: - Effective communication skills written, verbal and no-verbal with the ability to adapt communication for individuals with communication difficulties; - Able to function effectively as a multi-disciplinary team member; - Able to make clinically reasoned judgements when working with a variety of conditions; - Able to work autonomously, and ability to know when to seek appropriate advice / guidance from senior staff; - Evidence that can demonstrate problem solving skills in a relevant clinical setting; - Effective time management and caseload management in a relevant clinical setting
Desirable: - Experience of the application of health, safety and risk management policies; - Working with a range of professionals in Health or Social Care; - Relevant recent experience in a rehab environment; - Relevant experience in an acute or community setting (either student of post graduate) demonstrating a broad range of clinical undergraduate experience
