Are you looking to progress your career? We are looking for an experienced nurse to work within our Urgent Community Response Service (UCR)

Work as an advanced Nurse delivering high level clinical assessments & reviews to patients in their own home, implementing a comprehensive plan of care to prevent unnecessary hospital admissions

Involved in the delivery of care for patients as part of ongoing developments including UCR response standards & out of hours provision, supporting virtual ward patients

Work as part of a wider MDT of AHPs, specialist nurses, primary care, acute & social care colleagues

Monitor & improve standards of care through supervision, clinical audit, evidence-based practice, learning & development, & provision of clinical leadership

You will be expected to work across a seven-day week shift pattern including weekends & bank holidays

The shift pattern predominantly includes days & afters, but you may be required to work occasional nights

You will need to be able to travel independently between sites & to patients homes

All employees of the Trust are strongly encouraged to have their up-to-date flu vaccination to protect staff and patients.

Be an experience nurse, have completed the Nurse Prescriber course & Advanced Physical Assessment & Consultation Skills (APACS) qualification

Demonstrate a broad range of specialist nursing clinical expertise supporting high quality person-centred care. This role requires resilience & ability to problem solve quickly & effectively

Be expected to provide & deliver cover across our neighbourhood footprint, working in an integrated way with therapy, community nursing & acute, adult social care as part of the wider system

Work autonomously & be able to suggest & contribute to improvements within the pathway/team

Contribute to the on-going development of clinical practice & standards

Have a key role in training & education, participate in regular supervision & appraisals contributing to clinical leadership facilitating a culture of learning/reflective practice

Have line management responsibility for staff with health & wellbeing a key consideration

The Neighbourhood Teams Advanced Nurse (ANP) will be responsible for the delivery of safe and effective nursing practice across our Neighbourhood Teams, ensuring a 2-hour response to crisis call outs, providing ongoing support as appropriate.

Working as part of the multi-disciplinary urgent community response service to support unwell patients in domiciliary care homes and their own homes. To undertake face-to-face assessment, diagnosis and treatment for community patients presenting as unwell to prevent hospital admission, where appropriate and keep patients safe within their own homes.

The Advanced Nurse (ANP) will ensure clinical systems are in place to support and monitor practice and performance and support the Clinical Team Leader in the operational delivery of the service.

The Advanced Nurse (ANP) will hold accountability for all service user allocations across the Crisis 2 hour nursing pathway having shared responsibility for the clinical caseload management for the team.

Working with other lead Nurses the post holder will provide trust wide leadership and support within their own discipline or urgent crisis response.

• Providing specialist nursing assessment and intervention within individuals professional expertise to an allocated caseload of more complex cases, requiring a 2-hour crisis response and potential support or signposting.
• Utilising specialist and advanced knowledge of nursing practice to guide intervention and assessment methods appropriate to individual service user needs.

Practice as an independent prescriber within the bounds of current legislation, independently prescribing medicines and appliances where appropriate and within scope of practice.

• Working with service users to identify goals as part of the overall care plan, using assessment tools based on current evidence based best practice.

Having the flexibility and skill set, with appropriate training, to work across planned and Urgent crisis pathways supporting patients to remain well in their own homes, or long-term place of residence and / or prevent hospital admission.

• Supporting the culture and implementation of service user led care plans to achieve therapeutic goals.
• Ensuring the nursing team monitors, evaluates and modifies treatment in order to measure progress and ensure effectiveness of intervention.
• Showing a working knowledge of current legislation e.g., Urgent Community Response standards, safeguarding adults/Vulnerable Adult, Mental Capacity Act, Mental Health Act 1983 etc.
• Ensuring that the nursing team keep abreast of new approaches and treatment developments, ensuring that staff establish and maintain effective documentation processes in line with Trust and Professional standards regarding Clinical Record Keeping.
• Utilising resources effectively to resolve complex clinical and non-clinical situations.
• Broadening clinical research and development skills by leading on the evaluation of the service via service evaluation, audit and research projects to promote evidence-based practice.
