Job description

We are seeking a Care Co-ordinator (Level 1) to join our Living Well Team. You will deliver high-quality, person-centred support to adults with physical and complex needs, including enteral feeding (PEG) and Moving and Assisting support. Working closely with health professionals, families, and colleagues, you will ensure each individual receives sensitive, dignified, and personalised support.

  • Implement and review personalised care plans
  • Provide enteral nutritional support
  • Support the Care Co-ordinator (Level 2) and Team Leader
  • Assist with appointments, meals, domestic tasks, and personal care
  • Administer and record medication
  • Support with money management and household shopping
  • Promote community participation and meaningful activity
  • Provide emotional support
  • Apply active support and PBS (training provided)
  • Support with holidays and short breaks

These principal responsibilities are not an exhaustive list of duties, only a general guide to the role. In consultation with the post holder, the duties may change from time to time to reflect the changing needs of the customer/service.
