Make a meaningful difference at a pivotal moment in someone’s journey. This rewarding role offers the opportunity to support people in hospital as they prepare to leave inpatient care, whether returning to the comfort of their own home or moving into a care setting for continued rehabilitation, recovery, reablement and assessment.
As a Community Support Worker, you will be part of a dedicated multi-disciplinary team, working alongside therapists, nurses, doctors, individuals and their families to help shape safe, person-centred discharge plans. Every day brings new challenges, new experiences and the chance to have a lasting impact on people’s lives.
Our Hospital Discharge Team provides a responsive and compassionate service to Leicestershire residents across the acute hospitals. We thrive in a fast-paced environment, embracing teamwork, creative problem-solving and adaptability to achieve the best possible outcomes. Based at the Leicester Royal Infirmary, our Community Support Workers work across all three acute hospitals in Leicester, helping people regain independence and move forward with confidence.
We are looking for passionate and committed individuals to join our Hospital Discharge Team as Community Support Workers (CSWs).
This career-graded role offers a structured progression route from Grade 8 to Grade 9, supporting adults, their families and carers in line with the Care Act 2014 and Mental Capacity Act 2005.
At Grade 8, you will begin your journey with a comprehensive Learning and Induction Programme, including the Care Certificate and Diploma in Care Level 2. With mentoring from Senior CSWs, you’ll develop the skills and experience needed to progress to Grade 9, where you’ll take on more complex responsibilities including assessment, support planning, and case management.
