Job description
We are recruiting to a permanent 8A Complex Care Manager. The complex care manager is a member of the Proactive Care - Mental Health Learning Disabilities and Autism team within the strategic commissioning directorate with responsibility for delivering all functions related to individually commissioned, jointly funded packages of care across NEL. This includes but is not limited to packages for children with special educational needs, adults with learning disabilities, neuro-diversity and mental health needs and other complex presentations, it does not include continuing healthcare. Many of these packages will be joint funded with the local authority.
The postholder will work closely as a team to build commissioning plans and pathways to enable a shift away from spot purchasing towards pathways of care which emphasise a move to living as independently as possible within the community, accessing universal provision where possible and reducing the risk of ongoing institutional care.The post holder(s) will report into the Head of Complex Care and the team will be supported by two co-ordinating roles.
The post holder will hold delegated responsibility for a defined complex care portfolio (inner or outer north east London, babies, children and young people (BYCP) or Adults), leading the commissioning, review and quality assurance of individual packages of care. Working within a system governance framework, the role will ensure that packages deliver high-quality, person centred outcomes, support de-escalation where appropriate, and represents value for money, while contributing to wider service and pathway development. The postholder will be responsible for ensuring the ICB is appropriately contributing to the development of Dynamic Support Registers and any successor arrangements. The team will be required to keep abreast of changing legislation and statutory guidance and to ensure their work is informed by NICE guidelines and by best practice. They will need to work closely with local government colleagues to build consistent and coherent joint commissioning of individual packages of care, with Place Delivery Teams and with the Heads of SEND co-ordination.

Education / Qualifications
- Educated to masters degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
- Experience and proven expertise in working with people (either children or adults) with complex needs requiring placements
- Experience in relevant disciplines e.g. project management, change management, finance, contracts, estates, communications
- Highly developed specialist experience of working to develop individual packages of care in either a health or social care environment with multiple partners
- Deep understanding of the public sector, particularly health and social care, including all relevant policy drivers
- A good understanding of governance, assurance and risk management in a high cost and high risk area
