Job description
An exciting opportunity has arisen for a Band 6 Diabetes Inpatient Specialist Nurse (DSN) to be at the forefront of driving up standards and quality in diabetes care within the Inpatient areas of Liverpool University Hospitals (Aintree Hospital) and The Walton Centre. Aintree Inpatient Diabetes Service has recently achieved DCAP accreditation. Part of the role is therefore aimed at continuing this success by participating in audit of the care given to patients within Aintree Hospital to suggest improvements in services to improve the overall quality of care and patient experience.
The post holder will be required to deliver complex educational packages to health care professionals and patients around effective diabetes management.
The role will also include some development opportunities working in a variety of other clinical areas, outpatient clinics in the Diabetes Centre. It will include sub-specialist clinics e.g. pre-operative assessment clinic at The Walton Centre. The successful applicant will preferably have some clinical experience of working within the field of Diabetes and with awareness of and ability to work to evidence-based guidelines appropriate to the specialty.
The successful post holder will have the support of the wider Diabetes team to help in their professional development, Nurse Consultant, Diabetes Specialist nurses, Diabetes specialist Dietitians, Podiatry team and Consultant Diabetologists.
Diabetes Nurse Specialist will act as the clinical expert and professional resource to the multi- professional team, patients and carers within their Speciality Service.
The post holder will have a responsibility to develop the specialist area of practice in partnership with other members of the speciality team will work collaboratively to develop services and improve the quality of care delivered.
The post holder will carry responsibility for either a defined case load or offering "highly specialist service advice" this will include setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care, management and education.
The post holder will contribute to achieving the objectives of the clinical service group / speciality corporate objectives and work within the clinical governance framework.
This will be achieved through the core dimensions of the role, clinical practice, leadership, acting as the patient advocate and through audit and education.

The role of the band 6 CNS in Diabetes includes the following: -
- Perform specialist assessment of people with diabetes. Plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills.
- Undertake the diabetes competency programme of education based on the national TREND Diabetes competencies.
- Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation with the support of the band 7 CNS and the wider MDT.
- Co-ordinate and manage patient reviews from the Diabetes Dashboard or other referral systems.
- Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
- Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery and to support safe discharge.
- To support, assist and encourage the multi-disciplinary team in the review of people with complex needs, by the provision of new and innovative models of case management.
- Takes responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements.
- Participates in the supervision and the teaching of pre and post registration students, new staff members and junior medical staff. Providing in-reach education to clinical areas to support safe diabetes patient care.
- Contribute to the development of educational programmes in relation to diabetes care.
- Identifies training needs and develop training resources for nursing staff to enable role development and to meet the needs of the service.
- Take part in diabetes awareness programmes and national diabetes awareness activities to promote and support diabetes education and safe care across the organisation.
