Do you want to make a real difference to a persons life? As an Integrated Therapy Coordinator working within the Joint Community Rehabilitation team, you can do just that!
Joint Community Rehabilitation Service (JCR) is an integrated countywide domiciliary service delivered by Adult Social Care (ASC) and East Sussex Healthcare NHS Trust (ESHT). We have recently been Highly Commended at the Great British Care Awards. Our Integrated Therapy Coordinators work in partnership with therapy and support staff to establish peoples' needs and identify goals for them to work towards.
The role will include working in partnership with therapy and support staff to establish peoples’ needs and identify goals for them to work towards. You will use your organisational skills to coordinate the deployment of therapy staff and ensure that all relevant assessments are completed. You will assess and reduce risks, allowing clients to remain as safe as possible in their own homes, using a reablement approach. The role will oversee the provision of daily living equipment to help people maximise their potential and promote their independence in their home.
We will support you to develop the skills you’ll need to manage the challenges of this role which can be fast paced and require you to work collaboratively with professionals within our service and from external / partner organisations
