Transfer Of Care Coordinator (Home Care)

As Transfer of Care Co-ordinator, you will be a key operational link between health and social care services, ensuring referrals are processed effectively and care provision is coordinated seamlessly.

• Coordinate transfers of care from hospital and community settings
• Lead daily service huddles to identify priorities and manage demand
• Allocate and deploy Home Care staff through electronic rota systems
• Work closely with the Transfer of Care Hub and multidisciplinary partners
• Monitor service capacity and identify solutions to meet demand
• Support service performance reporting and funding-related data requirements
• Provide coaching and operational support to administration colleagues
• Liaise with service users, families, care providers and professionals
• Help ensure every service user receives timely, high-quality support

We're looking for someone who thrives in a fast-paced environment and enjoys balancing priorities while maintaining excellent attention to detail.

• Experience working within health, social care or a related care environment
• Strong organisational and coordination skills
• Excellent communication and relationship-building abilities
• Confidence using ICT systems and electronic scheduling tools
• The ability to analyse information and make informed decisions
• A proactive and solution-focused approach
• Experience working with multidisciplinary teams and partner organisations

A Level 2 qualification in Health & Social Care (or equivalent), or a willingness to achieve this, is essential.
