The Hospital Discharge Hub sits within the Integrated Adult Social Care, Commissioning, Communities, and Culture directorate. The service is made up of qualified Social Workers, and unqualified staff who work with system partners, individuals, and their carers to facilitate discharge where possible back to the person’s usual residence.
The service is a fast paced, dynamic environment, with the opportunity to work across different disciplines. You will play a key role in shaping and developing the service, and system relationships.

We're looking for an enthusiastic and motivated Team Manager to lead our Hospital Discharge Service. This is an exciting opportunity to develop your management skills, drive high-quality practice, and make a real difference in supporting timely and safe discharges from hospital.

- Lead, support and supervise a team of social care professionals.
- Oversee safe, timely and person-centred hospital discharges.
- Manage performance, quality assurance and service delivery.
- Provide professional advice, guidance and decision-making support on complex cases.
- Build strong partnerships with health colleagues and other agencies.
- Ensure compliance with statutory responsibilities, policies and procedures.
- Drive continuous improvement and contribute to service development.
- Social Work or Occupational Therapist qualification with a current professional registration.
- Significant experience of working within Adult Social Care and/or hospital discharge services.
- Experience of supervising, supporting or managing staff.
- Strong knowledge of relevant legislation, safeguarding and discharge pathways.
- Proven ability to manage complex situations and make sound professional decisions.
- Excellent communication, partnership working and relationship-building skills.
- Ability to prioritise workloads, manage performance and deliver outcomes in a fast-paced environment.
If successful, this role requires a DBS and you will have to apply for an enhanced Disclosure.
